I'll offer up my experiences with prophecy in worship although I know there
are others out there with more leading experience than myself.
If a word or tongue or exhortation etc... is given in our church it is often
during or just after the worship.  I recall only a time or 2 that this
happened after the sermon (say as a prelude to the alter call and when I say
this I mean by someone other than the pastor).  The worship leader will
usually affirm the word or encourage the congregation or what ever is
appropriate.  Since I'm not reeeal experienced in this I have at times just
looked to the pastor to say "now what?" and in his gracious way lets me "lead"
where ifeel we should be going (heavy responsibility at times).
Also, about being used in prophecy, I have been, but it seemed to flow out of
the leading.  Also, as a support singer (worship team member) the Lord would
speak through one of us and then the leader would direct or lead the
congregation to do what ever was spoken (maybe come to the alter for healing
or step out to someone and give or offer forgiveness etc...). We are supposed
to desire these gifts and I do.  I especially have the desire to "sing the
song of the Lord"-- sing prophetically a word from the Lord.  I have done this
some in private in my prayer time and it is somethign that I feel sure the
Lord is going to do in my life.  It's just a matter of "when" and being
obedient when He speaks.
I don't know if there is a given formula for what to do, I guess like all
things we need to let the Spirit of God lead us.
