Actually, the C9 would be C - E - G - B-flat - D. You're correct, the 9th (in general
usage) implies the 7th as well. A Cmaj9 would be C - E - G - B - D.
There have been discussion about this before, and I can't prove anything, but this is
what I've generally seen in some 30 years of guitar and keyboard playing. I've never
actually seen the conventions written down anywhere formally - does anyone know of
any "proof texts?"

If I stand, let me stand on the promise that You will pull me through,
And if I can't, let me fall on the grace that first brought me to You.
If I sing, let me sing for the joy that has borne in me these songs,
But if I weep, let it be as a man who is longing for his home.
