Not quite correct. A basic chord is composed of the 1st, 3rd, and 5th
notes.

Thus the base chord for C is c e g

Inverted chords are when you move the 3rd or 5th notes into base
note position.
Alan Wong, if you are out there, please correct me if I'm wrong, but
I believe that the 1st inversion of the C chord is e g c and the 2nd
inversion of the C chord is g c e

I always go back and forth between different inversions when playing
the piano. I use them all of the time when playing piano because
it is all laid out there before me. As for the guitar, I don't deal
with
them much because I lead the singing while playing guitar, so I try to
keep the hand movements as simple as possible for chord configurations.
However, for special music I always play inverted chords and
new progressions on the guitar.

C/ D is simply the C chord (I usually play a 2nd inverted chord) with
an additional D base note.

They are usually used in different kinds of chord progressions.

One example is:

         D D/F# G D A
The steadfast love of the Lord never ceases

There is another very beautiful progression in the middle
of a song called "I cry out" (I think that's the name).
The progression is somehing like:

Em Em/D D/C C

I don't have a keyboard here to check it. This is one of my
favorite runs to play because of the buildup of energy that
it provides in that song.

Sorry I can't remember what we call these kinds of Em/D & C/D
chords.
